We had a company update our web site a couple of years ago and they used your forms on two contact pages on the site.  The company who created these is no longer in business.

    Hello Rob,

Since you do not have access to this account nor you have access to the person who has access to account, here are the steps I would suggest you:

#1. Sign up for a free JotForm account with your email address: SignUp.

#2. Clone the form which is embedded in your website. The following guide should help you in form cloning:  https://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL

#3. The direct URL of your existing form which will be required for cloning:  https://www.jotform.com/form/1253642524

#4. Grab the desired embed code of your cloned form and use it to embed it in your website. The following guide should help you how to embed form in your webpage:  https://www.jotform.com/help/34-Adding-a-Form-to-a-Web-Page

     Thank you!
