We use a form to gather updated info from our volunteers on a yearly basis. We also ask some questions to test their knowledge to make sure they remember important information that they learned in training. Is there a way for the form to tell them if they selected the correct or incorrect answer?

It is possible to create a quiz forms using the Quiz Form app with your form. Please refer to the guide below that can help you with using the quiz form app.

Hello, I've been working on this and when I try to convert a form via the Quiz Form app, some of my questions don't appear as options. They do have conditions on them so I wonder if that is why??

What are the fields that don't show up? Only fields such as dropdows, checkboxes, radio buttons, will show up.

Yes they have radio buttons. I did some troubleshooting and it's because they have conditions on them. I removed all of the conditions and was able to format the quiz then add the conditions back in.
The only issue that remains is if I set a condition for a question to not show up based on a previous answer given, it still shows up at the end with all of the correct/incorrect answers. Not good since it's a question they're not supposed to see to begin with.

I am able to replicate the issue that you are referring to. As I tested on a separate simple form with two fields having Show/hide conditions. The hidden fields on the form are not displaying in the Quiz Form app to check the correct answer.
Let me forward the issue to our backend team to take a look. Once we have any information in this regard, you'll be posted here.

Ok, thank you. To clarify, the question I'm referring to will need to be hidden for some and shown for some, depending on what they answer. If I set up the quiz before applying the conditions, then go back and apply the conditions, the question show up at the end when the correct answers are shown regardless if it's supposed to be hidden or not.
If I apply the conditions before setting up the quiz, the questions with conditions don't even appear in the set up portion of the quiz, so they then don't show up in the end with the correct answers.
