Submit Button is not working, just says "Please Wait"

When you click submit it simply says "Please Wait" and never submits the info and never sends the visitor to the thank you page.

We've also tried going directly to the form hosted by jotform and tried filling it out there and it does the same thing. Here's the direct link to the form here

I have resolved this issue. I had my payment settings set to user defined amount with suggested amount turned on, which should be off.
