Why are my forms diasabled?

I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. The account was suspended due to the form https://form.jotform.us/71238399584167 which is collecting credit card information on plain text fields.

Unfortunately, it is not allowed to collect credit card information on plain text fields using JotForm. Such forms are considered as phishing forms and associated accounts will be automatically suspended. Please refer to our terms of usage for more information on phishing. If you need to receive payments, it is required to use one of the payment methods available in the form builder. These payment methods comply with the PCI/DSS security standards to process the payments securely.

I have now removed the guilty fields from the form and activated your account. Please refrain from adding such fields to your forms to avoid any further suspensions.
