I cloned your form and I can see that you have enabled the Unique Submission and Unique Question feature for your form. This will prevent multiple submissions to your form. To disable it, please follow this guide:
1. Login to your account and open the form.
2. Go to the Settings page and select the Form Settings tab.
3. Click the Show More button4. After that, choose the "No Check" for the Unique Submissions drop down menu. Also, choose the "No check" again for the Unique Questions drop down menu.
Hope that helps. Please contact us again if you have questions. Thank you.

Yes, it did help, it’s solved.
Thanks a lot.
I have been enjoying a lot your software and seriously thinking about upgrading the account to be able to do more with it.
This answer and support from you makes me more confident about it.
Next step is to learn how to be able to create payments within a form. Probably will need help again ;-)
Best regards and thank you, again.
