How to add columns to a form?

I would like to recommend you to use "Input Table" element. You can also change the input type to the text box.

Another way I have found is to load a text box, on the right you have the option to create a box with as many columns etc as needed. This, of course, doesn't allow an entry by the student but does work very well.
