Hidden Fields are shown on the form

    My form is showing hidden fields.  Any ideas?

    Thank you for contacting us.

    I have cloned your form in order to test on my side. I could see that you have set the Conditions for Show/Hide fields. You must set your conditions again for solving your issue. Here is a condition that you must re-set it:

    Please uncheck these fields:

    16.Get Date

    17. Day of the Month

    22. Calculation Field(id34)

    Your  form will be shown like that:
