Conditionally hidden fields are having values cleared

I have some insert conditions as follows:

When I test the form, and fill in the first set of fields, as follows:

then nothing happens in the second set of fields. I was expecting them to prefill with the same info above:

Can you tell me what I'm doing wrong?

In your forms setting the option to clear conditionally hidden field values is enabled:

Set this to "Clear on submit" or "Don't Clear" and your conditions will work as expected.
