I haven't changed anything on my forms and haven't recently added any new css styles on the back end, but it appears something done behind the scenes--I'm guessing on your end--has created an issue on the layout of my forms...
The Full Name and Address fields on my forms are no longer showing up as they are supposed to (see screenshot). The first name and last name fields should be side-by-side, but appear on top of each other in the live view. They are also smushed together with no vertical space between them. The address field is vertically smushed as well, with City and State having no space between them.
I haven't provided a specific page because it is true across all of my forms.
Can you look to see what recent changes have been made that would cause this to happen to my forms? I haven't changed anything on my end and all of the sudden the layout is wonky.

Thank you for reporting this issue with us about your forms suddenly not appearing correctly.
As a general rule of thumb we will never touch your forms without your permission but, its certainly a possibility that a change on our back end could have thrown something out of whack even though it should not have since the css involved is usually the same across the board.
Can you please provide us with a link to the example form in your screenshot so that we can take a closer look at this problem?

Thanks, yeah I didn't intend to sound like you had come in and directly edited my form code... but maybe you guys changed a theme or something on your end which effected things across the board on my end.
Anyway, here is one example:

The weird thing is that from what I can tell (I haven't gone through every detail of every form), it only appears to be effecting the Full Name and Address sections. Other items on my forms are spaced as they should be.

Not really sure how that happened, did you check the Revision History for any CSS injection that may have caused this issue? https://www.jotform.com/help/294-How-to-View-Form-Revision-History
If there is nothing there, you can fix this issue on that form by injecting the following CSS code: https://www.jotform.com/help/117-How-to-Inject-Custom-CSS-Codes

This is not really an acceptable answer.
I have not made any changes to these forms. As stated at the beginning, this is across ALL my forms that have full name or address fields. I didn't make any edits or any new injections. The form layout was fine one day and not the next... and it's a global change however small that happened here, not something I inserted on my end.
Your solution suggests that I go through all of my forms and add this new code to each one which I find a bit troublesome. This is not a solution, it is a mask... it doesn't explain or fix what caused the problem to begin with, just covers it up with more code.

We are sorry for the inconvenience this has caused you.
Since you have mentioned that you didn't make any edits on your form but the issue on form layout across all your forms suddenly appears, I will raise this issue to our backend team for further checking.
For the meantime, my colleague's suggestion is the only way in order for your form to be fixed.
However, rest assured you will be notified in this thread if there are any updates from our backend team.

So... I hadn't heard back in a while and started doing some deeper digging around. I figured something out and maybe you guys can give it a look and see if you can rework this...
The name and address fields have "sub-labels." For the purpose of my forms--pretty much since I started using this platform--I have chosen to remove the sub-labels on the name and address fields and use placeholder text instead. In the past, this didn't cause any issues, but you guys changed something recently, and now empty sub-labels removes this key ingredient:

Without that "form-sub-label-container" class wrapped around those fields, the layout goes wonky as you can see in my previous screenshots. For some reason important elements like "width: 48%;" and "margin-right: 4%" get totally removed without that class.
Now if I have to, I can go in to all my forms and add a space to each sub-label so that they are there even though they are not showing... but I'd rather not need to do that. For one, I preferred the tighter vertical spacing not having them afforded me, esp. in regards to the address fields. It also seems to me like whatever code elements set up the address fields and name fields to show up properly before should be maintained whether or not there is a "form-sub-label-container" class placed on it. The fact that there is or isn't a sub-label shouldn't change the width of the field or whether it shows up side-by-side as intended or inexplicably smushed on top of each other.
Anyway, the sub-label issue may not be the whole cause of this, but it should provide you guys a place to look and see if you can please fix this for me.
