How to find this form so it can be edited?

    Glory Lutheran Church Facility Rental Application

    We created the above form and it needs to be edited.  Unfortunately when I login I cannot locate it so I can make the changes.  Would appreciate your assistance.  Thanks.

    The form was under a guest account. So I moved it under yours, you will be able to find it under My Forms section.
