1) If you would like to delete the Square integration from your form, just delete the Square payment field.
2) If you would like to disconnect your Square account from the Square payment field, go to the payment field settings, and switch the Mode from Live to Test or vice versa. It will disconnect your account and let you to authenticate again if needed.
