I am using the customisable list widget and want to add field to 2nd row

1) Hi rather than making the form too wide i wanted to add  a comments field below. Is this possible?

2) This is a form  that our trainers would use and update weekly is there away to retain the data for the user as it will not work if they have to fill in the form from scratch each time

1) I can't find the form you are referring. I checked the form and it seems that it is deleted:

2) You can create sessions for your users and they can continue to fill the form later by their email address. They can submit it once in a month and they won't need to fill the forms from scratch. Here is a guide on how to do it:
