Lightbox displaying two forms on my webpage

Hi Jotformteam,

I tried to embed a Lightbox form in my Weeblywebsite. www.medpartner.eu

But it does not work well.

1. Problem: Always when I click the Button of the form "JETZT BUCHEN" the form is covered by a grey field? I can click that away, so I can find the form under it.

2. Problem: The form is displayed twice? Especially this problem appears on mobiledevice.

I have checked your web page and see that there are two forms embedded on the same using the light-box embed method. Please try removing the following section from the second embed code in the web page and see if that helps.

So, the second form embed code would be as below:
