Move my Jotforms to another account

Hello, I would like to move my jotforms from the username joand to the username metroaging.  I would like to move the following forms: Cleaning up MAAA, Quarter 1 baseline survey, Quarter 4 baseline survey, POST Survey for ALL programs, 2016 - Quarter 3 baseline survey

I have moved those 5 forms from your account to the user metroaging.  The forms are now available from that account.  If there is anything else we can help with, just let us know.
