My account is autosuspended

    Hi Team,

    While logging i am getting error cannot login.autosuspended.

    Request you to please reactivate my account.

    I have checked your account and your form. Your form contains login credentials. You can not directly get personal sensitive information such as password, credit card details. Because it violates our terms of use. If you insist on collecting any of this information, your account will be suspended again.  

    I have deleted this field and re-activated your account. Your form is enabled now, you can log in to your account.
