You can easily identify a person who filled out the survey form by using Jotform's page redirection feature.
To do that you can follow these steps :

1. First, ensure your form has an Email field where users can input their email address.

2. Go to your form's settings and select "Thank You Page" from the left sidebar, then you have to choose "Redirect to an external link after submission" where you will have to input the URL to the website you want to redirect the users.

3. To pass the submitter's email address as a parameter you'll need to add it to the URL you're redirecting to. You can do this by adding ?email={email} at the end of the URL. Just a reminder to use your unique name at the {email} for the email field in your form. You can find the unique name by clicking on the Email field inside the Form Builder.

4. Whenever someone fills out the form and submits it, they will be redirected to the external link where their email address will be attached to the URL.
