We received notification on 5/20/17 at 9pm from a user that our form was not working for them. They completed all of the information and required fields and then when clicking on the submit button, it would not submit. I tested the form from the user standpoint and found that the same thing happened to me -- clicked the submit button and nothing would happen. No error message, no submission confirmation, nothing. I did a test submission to your form and I was not able to replicate the issue. Here's the Thank You page of your form:
I also cloned your form and it is working fine on my end. Please try updating your widget. See screen animation below:
