User are having problems with our form, submit button not working

We received notification on 5/20/17 at 9pm from a user that our form was not working for them. They completed all of the information and required fields and then when clicking on the submit button, it would not submit. I tested the form from the user standpoint and found that the same thing happened to me -- clicked the submit button and nothing would happen. No error message, no submission confirmation, nothing. Please advise / fix:
