How can I add a sub label to my question?

I would like to add the sentence "You are eligible for 250% tax deduction." under my questions, "Yes, I would like a" and "Yes, I would like".

It should look like how the Name forms has "First Name" and "Last Name" in small text.

Unfortunately, you can't add a sub-label to the Multiple Choice field. However, you can add a Text field on the top or bottom of the Multiple Choice field. 

I checked your form and it looks like you already added a Text field.
