Is it possible to create a single form with unique pages for each of my customers?

I need to create a unique form and unique page for each of my 500+ customers

This page needs to have 5 or 6  html links at the top but the links need to be created automatically from my online CRM. The form also needs to automatically be sent to my customer's mail to. Also pulled from my CRM. 

Is this possible with Jotform? 

You need to subscribe to our Gold Plan to create more than 500 forms. Please check our pricing page for more details: https://www.jotform.com/pricing. But if you mean just a single form with 500+ pages, then I don't think that will work since that makes the form so huge and will slow down its performance.

I would suggest creating a standard form. Add the fields, but make sure to have a section where each client can enter their details.

As for the links, unfortunately, it's not possible. We don't have a feature that would allow you to post links in the form from an external source. 

As for sending the form, I would suggest doing it in your CRM, not unless you don't have a way to send a bulk email to your leads/customers then you need to do it manually. Maybe, a newsletter using third party programs like MailChimp.
