Form Not Fully Responsive When Parameters Passed in URL

I use my form located at form.jotform.us/71168406898166 to pass various variables to the form and use the following as a forwarding address via one of my domains:  form.jotform.us/71168406898166?input44=Denise%20Stephens&input46=101256&input73=whyw8@sbcglobal.net&input245=330987&gboPhone=(817)%20988-1086.  

The domain I forward the aforementioned url is located at offthehealthinsurancegrid.com.  When I text someone just the jotform form url with no parameters, the form is totally responsive on a mobile device.  However, when I pass the form with the variables in the url, the form is not responsive on a mobile device.  Can you offer an explanation and please advise what I need to do to force the url with parameters to be fully responsive on all devices?

Never mind.  I resolved it.  It was due to masking turned on on the forwarding domain.
