Help with conditions + hiding fields

So in my form: https://form.jotform.co/70396515599874 I have a checkbox image widget that displays several fields depending on which image you checked. The conditions work fine and display the correct fields but the problem is when you un check the image the fields that it displayed before remain on the form. Can you help me set that when you uncheck the option the fields that earlier displayed with the conditions stated, can hide?

I have tested your form, but I am not able to replicate mentioned issue. When I un-checked selected images, conditional fields were hidden.

Yes they hide because you didn't answered any of those questions. If you answer one or more questions of those and by any reason you decide to uncheck the checkbox then some fields stay.. can you try answering the questions that the image checkbox display and then try unchecking?

Thank you for providing us additional information. I was able to replicate your issue this time. 

This issue is related to conditions that you have set on your form. Fields that are shown, when image is unchecked are conditional and they will be shown if other fields are filled. When you uncheck the image, field will no longer be shown on the form, but it will still be filled and that will trigger conditions, to show other conditional fields. 

To resolve this issue inside of settings page of your form set Clear Hidden Field Values to Clear On Being Hidden.
