    Sorry, I had an error message when I clicked on the Post Answer.  My question is: I created a check box and text box form for my website.  I then embedded the code from Jotform.  I went to my site to test the form, checking off boxes, and adding text and clicked the SUBMIT button.  The e-mail came in with out any of the check box results - just the fill-in the text boxes.

    Please tell me how to create check box for, "Check all that applies?" So that the e-mail I receive gives me all the results?

    When I ran a test submission on a clone of the form, I actually got the opposite result from one of its notifications: Only the checkbox entries were displayed. Perhaps it would be best to just delete the notifications and replace them with new ones. Please try that and report the results.
