Quiz App: answers keep reseting

    If the correct answers are selected and form saved, the quiz only works once then the answer indicators return to blank. If the answers are added again, VIEW QUIZ RESULTS will show past entries correctly graded and cycle begins again. I've tried clear my cache and different browsers (IE 11 and Chrome). Also, the app adds a "t" in front of the correct answer on the review answers page
