There are two forms in your account and both request that users log in to social media accounts for different reason.  We do not allow for requesting such actions through forms and do not allow for the collection of login details for any reason.

Since both of your forms are against our terms of use, the account will remain closed.  If you would like to create further forms, please do not ask for any type of log in details or request that users login to any sort of account.
