Forms Disabled

    Both of the forms I have created have been disabled. I did some research and discovered it is because one form requested passwords. The client requested those fields, and I apologize as I did not realize this violated terms of service, they need that information as transaction coordinators to do their job– it is not a phishing scam. I will let the client know that we can't ask for that info.

    I have deleted the username and password fields that violate the TOS, please reactivate the account.
