I cannot login as my account is autosuspended

    I recently upgraded our company account and tried to add a user.  I logged out and then tried to log back in but my login no longer works.  I was also having trouble previewing any of our existing forms before I logged out.  The forms that we use on our corporate website are now disabled and can't be viewed or used by employees.  Can you please try to reinstate my login as soon as possible.

    Upon inspection I found that your account is indeed autosuspended because your form "IT Setup for New Employees" with ID 71515612675154 is collecting passwords, in violation of our terms of use.

    If you accept our terms of use, and grant us permission to remove the offending fields/form, we may consider re-activating your account.

    I went to the Terms of Use page but there was nowhere to acknowledge or accept them. We accept the terms of use and authorize you to remove whichever fields are in violation of the terms. Please make the changes as quickly as possible so our employees can have access to our forms again.

    Thank you for accepting our terms of use. I have removed the offending fields and re-activated your account.
