I made a contact form with JotForm a few weeks ago and put it on my Wix website via the HTML code, following the instructions given on your site.
It worked great at first, but now it won't show up AT ALL on my site! I can click and see the outline of the HTML box, and it gives me the option to change or update the HTML. I tried a few times to copy and past the HTML from JotForm. Still not showing up.
Hopefully there's some simple reason for this.

Your forms are actually disabled because your account with username True2YouPhotography has been suspended due to possible phishing.
It seems like your form titled "Invoice Submission Form" is asking for bank information.
Please note that we do not allow forms collecting sensitive information like credit card details, login credentials, passwords, bank information and the likes. These things can be used for identify theft. Below is a section from our terms page.
"Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information."
I have edited your guilty form and removed the fields that violated our terms, and I have re-activated your account, your forms should now be accessible again. Please review our terms page again and avoid anything that may violate our terms to avoid being suspended. If you need to request payments from your users, I suggest using our payment integrations.
