The following forms have been disabled on my web site:
Shed Horn Cellars Order Form
Shed Horn Cellars Wine Club
Can you please tell me why this has happened and how quickly they can be restored?
This issue is causing my small business to lose sales and revenues!!

You are using our forms to collect credit card account data.
You violated our Terms of Use. We now have suspended the account and forms.

Ok. Well, we've been using Jotform for online purchases for years without incident.
How can we remedy this for our client so they can continue to use Jotform for their small business?

You have credit card fields on two forms:

In order to activate your account, it is required to agree to our terms and remove the guilty fields on the forms. Please confirm so that we can delete the guilty fields and activate your account.

I have deleted credit card fields from your forms and I have activated your account.
Please note that card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields. The only services allowed to collect such data are those that are DSS/PCI certified.
If you want to have payment fields inside of your form you have to use one of JotForm payment tools.

It seems that your latest reply is not shown here on forum. Here is your reply that I received via email:

Will this solution allow me to connect sign up form to my Square account so I can bill for future purchases?
For example, there is no charge to sign up, but I will need to bill to credit card on file for future purchases."
You can charge your customer at later date, by enabling payment authorization.
How to Enable Payment Authorization
This option is available for Square Integration.
