I have the same form embedded on multiple pages but wondered if there was a way I could find out what enquiries came from what pages?
I don't want to create multiple forms because this will impact my reporting. I have linked you to our 'contact the team section' where each member page has a contact from.
Thank you in advance for your help.

If you want to know the current page URL where your form was submitted, I suggest you add a Get Form Page URL widget in your form.
The widget is not visible for your respondents to see. It detects the web page where the form is embedded and provides the page URL each time the form is submitted.
Thank you.
