I got a notice on my account that it had been disabled due to a possible phishing scam. I am now able to access my account, but my forms are still disabled. Can someone please let me know what I need to do to get this back up and running?

We have reviewed your account and found:
We cleansed the form and activated your account. They are available for you to continue developing your forms.
Please be aware that this is illegal because it's considered a phishing activity, which violates our terms. Card number, expiration date, security codes and card holder name are considered critical information. The only services allowed to collect such data are those that are DSS/PCI certified.

Thanks for the response. I apologize; I didn't realize there was a request for credit card info. I still cannot access my forms however; it's still saying "form not found, disabled". Email for account is email@example.com if that helps.

I checked your forms and I can confirm that there aren't any phishing fields in your forms from now on. Therefore, I have activated your Jotform account. You can access to your Jotform account and your forms.
