No matter what email address I use, I don't get a notification. I tried the "test" button, and I tried embedding it and test-completing it myself. Nothing. :/

For notification problem, try to change your Sender Name to JotForm, and your Sender-Email to firstname.lastname@example.org and then do a test?
You can also add JotMails ip addresses to your mail providers whitelists.
If this still doesn't work, please do let us know.
