That's possible but you will have to manually inject the CSS codes from the theme's stylesheet to the target form.
You can grab the theme's CSS here https://cdn.jotfor.ms/themes/CSS/54a64d6e25d7909c1f8b4567.css?themeRevisionID=5628e855977cdf35308b4567.
This will only give you 90% of the styles since you will have to make adjustments in the form designer if you want to mimic the overall style.
Alternatively, you can also clone your existing form, this will give you the same layout and the fields. You can then make changes on the fields in the form builder whenever you want Guide: http://www.jotform.com/help/27-How-to-Clone-an-Existing-Form

Hello,
As my colleague EltonCris has suggested, you can clone the form in order to have the same layout and modify it as you can see in the following guide: https://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL. This is the best way that will work 100%.
I have tested injecting the CSS codes that my colleague EltonCris has supplied above to a default form and I have seen that the form has appeared as seen below:
So, you need to adjust the form more in Form Designer after injecting those CSS codes.
I recommend you to clone the form and modify it.
Thank you.
