I checked your account and was able to see the issue, I also tested this on my own forms and it seems like the changes made on the form are not being logged into the "Revision History" list. I'll forward this to our developers to have it checked.

We are sorry for this inconvenience.
The issue should be fixed now. You will be able to see changes on your revision history after that. However, we couldn't recover the lost revisions.
