I looked and saw my account is suspended. After searching for answers, I see others had an anti-phishing issues. I'm not aware any fields on my form have sensitive data. Can you please let me know the reason for the suspension.

Your account has suspended because you are using a form to get upwork work, while all Upwork jobs should be done and processed through Upwork.

Are you saying I cannot create a form where my VA from Upwork submits their daily work? It is a task they perform daily and the data feeds to zapier to podio so I can perform my work and pay them based on each task submitted. Upwork does not have a form that I can have them fill out for the task based activity that is the work they are performing.

I have checked your JotForm and it seems to be normal except for the usage of Upwork on the form. It is restricted to use any other company's identity on the forms unless you are using the official email ID to register the account and follow some guidelines. I have removed the word Upwork from the form and activated your account.
Please check now and you should be able to login to your account and view the forms normally. Please avoid usage of any company identities or collection of sensitive information using the forms that might suspend the account again considering phishing forms.
