Our previous webmaster created a form using Jot Form for our website www.burnscredit.com we are having issues with form and our current webmaster/provider can not access that form without a account password. This Form was created approx 3 years ago and that webdesigner is no longer in business and did not keep Jot Form access records.
He said he thinks that he created it under name burns ford and that he probably used email@example.com as a email address??
Can you provide us with access to this Jot Form account?? You may view form in question at www.burnscredit.com it is the Credit Application or http://secure.netsolhost.com/burnscredit.com/credit.html for actual form page.

The account name was burnsford and it uses this email account r###@####ign.com. You can reset the account password by going to www.jotform.com and click on the "Forgot Password?" button. Option to reset the password will be send to the email account.
