A Banner Ad from JotForm just started showing up on my contact us form

    Please advise why a banner ad from jotform has suddenly started appearing on our contact us form (see attached screen shot).

    We needs this removed as soon as possible.

    I have checked your account and found that it's in the Free status.

    Please note that removing the JotForm branding is an option reserved only for upgraded accounts. Accounts in the Free status cannot remove it.
