Hello, i have follow the guidelines and make sure there was no conditions set in place that prevented it and i also went to the submission settings and make sure to check for header and text to appear on fillable pdf, i have been trying to make this work since yesterday, i checked my form when i started and it was displaying the header texts but by the time i finish it didnt. I also made sure to not sue any widget on the form its all for them quick options menu. Can you guys please see what is wrong, thank you so much.Page URL:

I have cloned your form and I cannot reproduce the error.
However I'm guessing there is a bug somewhere related to your browser displaying the Headings.
In the builder the Headings are not defined (but the subheadings are). I know thats probably how you want it. But instead of leaving the Headings blank lets give them an empty space character.
Please get back to me and let me know if it didnt work. And we can try somthing else.

You could also try opening the PDF file in a different program. Adobe reader is excellent and also free:
