Hello, i have follow the guidelines and make sure there was no conditions set in place that prevented it and i also went to the submission settings and make sure to check for header and text to appear on fillable pdf, i have been trying to make this work since yesterday, i checked my form when i started and it was displaying the header texts but by the time i finish it didnt. I also made sure to not sue any widget on the form its all for them quick options menu. Can you guys please see what is wrong, thank you so much.

I have cloned your form and I cannot reproduce the error.
However I'm guessing there is a bug somewhere related to your browser displaying the Headings.
In the builder the Headings are not defined (but the subheadings are). I know thats probably how you want it. But instead of leaving the Headings blank lets give them an empty space character.

You could also try opening the PDF file in a different program. Adobe reader is excellent and also free:
