Account suspended

    My account will not allow me to login.  Showing suspended.  I know from reading it is possibly due to an old form that we used to process credit cards.  It is no longer used and I am happy to delete but cannot do so without access to the account.  Help me please.

    The email address is associated with the account username "cubsblue". The account is suspended because the form "Online Payment" ( is asking for credit card information using the basic fields. As per our terms of use, collecting sensitive information is not allowed in JotForm.

    We need to delete the guilty fields or the form in order for us to activate your account.
