I checked the Page URLs you provided and it is working fine except for the (www.ncbrof.com). Here are the internet browsers I used to open the website:

Are you referring to this Page URL instead (www.ncvrof.com)? If yes, then it is also working fine on my end. Also, I checked your account and form, it is in good status.
I did a test submission to all of the Page URL you provided and I didn't encounter any issue. Could you please try opening the website using the different internet browser and see if it will work?

I also checked your account and tested your forms. I found that they are all working properly. So please follow John's advice and try a different browser. Also you could check the forms again yourself. If there is any issue, please note it down and let us know.
