How to have more Date field using condition logic on the form

My users will be filling in forms to schedule tests. I would like them to have the option of scheduling multiple tests on one form. Is there a way to add more Date fields, but only if the user wants to schedule more? Like an "add another test" button and another set of "Start Date" and "End Date" fields pops up?

It may not be possible either, but I thought I'd ask. :)

Yes you can have multiple Date fields on your form and then only show them base on user's response using condition logic.
