    Sir/Madam,

    Purpose Of My Form : For Online Submission Of Some Requisition,Issued & Received Items 

    Yes I am try to use a specific password in my "e-Issued" Form to restrict unnecessary submissions

    I am using directly your Jotform link,

    But My Account Suddenly Showing Auto Suspended Kindly Resolve the problem urgently

    Presently i can not access any from out of 5 forms

    Urgently unlock my account and delete such fields those are suspended my account,And kindly intimate me which fields are not allowed in Jotform table .

    Further Requesting to Jot Form Team Kindly, Unlock My Account Urgently
