My form information is not sent to Google spreadsheets

The First time: row 1-513 is normally. 

When problem, it unlink and link again. old submission is sync to new file but new submission is can't update to sheet

I have tested your form. Please check if the submission is being recorded in the spreadsheet file? 

Please note that changing any part of the spreadsheet, especially the header/labels is not recommended as it breaks the integration. We also do not recommend moving the spreadsheet file into a different folder/location as doing so will also break the integration. I checked the forum and I don't see any reports related to this for the past few days so the problem might be specific to your form. 

By the way, would you also mind sharing us the current spreadsheet file? Please share it with read-only access. We need to view the file for testing purposes.

You seem to have removed the other columns in the spreadsheet. As I have mentioned, edits are not recommended since that will break the integration. Please try keeping the spreadsheet intact, all of the columns and see if the issue persists.

Can you update new form to use old url? or create redirect?

Because my web reference to old url many link (more 600 reference). if change url is big problem for me.

Unfortunately, you can not integrate with the same spreadsheet file, the new sheet file will create with each integration.

Maybe you could add (in the sheet that you shared already) a link to a new sheet with an explanation or something similar.
