    This account should be at FREE status.  We will not require more than 100 submissions per month.  Please do not bill the Mississippi Division of Medicaid in upcoming months.  The Mississippi Division of Medicaid will not authorize any additional payments for this account.  In fact we were under the impression that this was a one-time billing for our account and we DID NOT request or authorize additional months.  If at all possible we would like to request a refund or credit for the additional month of billing.

    When we need to expand our submission numbers again, we will contact your billing department at that time.

    When an Jotform account is upgraded, it is entitled to recurring charges either monthly or annually, so your account was re-billed automatically.

    Your account subscription is now cancelled and will not be charged in August 12 anymore. If you need to re-subscribe in the future for a few months, just let us know when the month period is about to finish in order for us to cancel it , so you don't get charged for the next period.
