I am not sure what happen to the form, but when I checked the paypal field using the payment wizard it display like this.
Please try deleting the paypal/product field,(it will ask you if you want to keep the product list, please answer Yes). After delete, add the paypal field, it will again go to the process using the payment wizard.
I tested this on my form, I was able to make it work here http://www.jotform.me/form/22176002587451
Please inform us if issue persist after trying the instructions.
