Blocked from my account for phishing

Hello, 

i recieve an email submission as i usually do and when i click the pdf attachment to view it, it opens a form that says "access denied Looks like your IP address has been banned because of a possible phishing attempt. If you believe that there is a mistake, let us know using the form below."

i dont want to fill the form out if its someone trying to phish me haha

I checked your account and it looks to be in good standing.  I did not see any other accounts associated with your IP address so I am not sure what caused it to be banned if it was.  I whitelisted your IP and the error message should no longer be present.  If it is, let us know your current IP and we can whitelist that as well:
