I received this email:
I have checked your account and see that it is suspended due to the password fields on your form http://www.jotformeu.com/form/62002071742341 . It is not allowed to create login forms or collect sensitive information like Credit card numbers, passwords, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.
In order to activate your account, it is required to agree to our terms and to delete the guilty fields on the form. Please confirm so that we can activate the account after deleting the guilty fields.
But I'm unsure how to proceed. I want to accept the terms and conditions or correct the errors but can't see how to do that if I can't login.
Can you advise?

Your account was reported by our phishing detection system as suspicious of phishing activities, your form "Submission form for BFI Future Film Festival 2018" is the one that triggered the suspension because of the password field you have in your form.
Jotform implements scripts that allow us to detect real scammers, our terms are clear regarding to such type of forms. Please read the following excerpts from our Terms regarding to Termination of the service and Scams.
Termination. You agree that Company may terminate your JotForm Service membership or suspend your access to all or part of the JotForm Service, without notice, if Company determines, in its sole and absolute discretion, that you have violated these Terms.
Scams. You agree that you will not use JotForm or any form(s) you create using JotForm for scamming purposes to attract, lure, or illegally obtain payment of any sort from others by unjustifiable means such as posting a JotForm as a money-making program on a classified ads site as an example of that. Any account reported or found doing so will be immediately suspended.
Kindly refrain from creating form with email/password, login/password or credit card details.
