When I download the form as a PDF it does not download the First Name field and therefore it is not seen when printed.

It's possible that the First Name field in your form is not included in the PDF format. To verify that, please go the form's submission page, click the PDF icon at the top and select "Customize PDF".
This will redirect you to the PDF Design page where you have the option to either reset everything to its default design state or just add the missing field. I suggest you do the latter.
The added field will be placed at the bottom, but you can drag and place it to your preferred position.
Please do not forget to click the Save icon after the changes and make sure it shows "Report Saved" before testing it.
