    Hi,

Your form is working fine in my end and I couldn't fine any problem on it. Can you please try to delete your browser cache and cookies to see the changes of your form?

    Thank you!

I think the changes are there but here is the problem-
For example, do you have a home phone #?
when I answer "No, who has a home phone # anymore" that answer does not come through on the form
The other 2 questions-the same thing happens

You have a custom Email Notification Template for that form (Notification with changed template body), but our system does not update such Email Notifications.

In order to solve the problem you can either re-create Email Notification (all the form fields will be included) or modify your current Email Notification Template to have these new form fields.

If you would like to modify your existing Email Notification you may follow the next steps:

    1. Open your Email Notification.

    2. Click on some cell, and then use an Insert row after icon to have some space.

    3. Type your field name (Question), and insert the form field {tag} using Form Fields menu.

    4. Do the same for other new fields.

Please feel free to contact us if you need any further assistance.

    Hello-
I have never had to do revise any of my forms this way in the past. Many, if not all, my forms have a custom email notification....
My webmaster helped me do it your way, and yes, the questions now show upon the form but they are conditional-how are you supposed to set up conditions in the way you have instructed??
Im not trying to tell you that you are wrong but.... I have never had to amend a form via the way you instructed.

Hello.  It is possible that you or your webmaster may have modified your email notification before.  This is because that once you have modified your email notification, it will no longer automatically update.  The fastest way possible for your new fields to appear on your notification is to replace your old Notification with a new one on your Email Alerts list.

If you wish to set up a conditional Email Notification, You will have to create another Notification email with the proper contents that you wish to have in the contents.  And then create another condition using the "Send an Email After Submission" option. This condition is designed to send an email to the chosen Notification.

If you have further questions, please do let us know.

    Thanks.

Im sorry but you are really losing me....
I have created and then amended all of my forms 100's of times and never via the method you are describing.
Why cant I amend the form the way I have always been doing it - the edit feature under My Forms???

It's possible that the ones you're modifying are those which the email notifications have never been amended.  Or can it be that you are editing your form with just having alignment settings or adding headers?

This is how it's been ever since and this process has not been changed:  Once you have edited the contents of your notification, it is somehow considered by the system as 'dirty' thus it will no longer automatically update anything in the notification contents when you add/edit fields to your form.

Let me share an analogy why this is done the way it is:  A possible scenario is that if a form owner modified his notification contents to his liking, an automatic update may completely ruin the layout and we can simply imagine the horror of the form owner once he finds out that the automatic update destroyed his effort.

Hope this clears your concerns over automatic field updates on the notification email settings.

Please do let us know if you have further questions.

    Hello-
Is there someone that can help me. I have no idea what the 2nd paragraph in your email wants me to do..."proper contents that you wish to have in the contents?'"

Liyam, was talking about conditional Email Notifications.

Mentioned second paragraph means that you can have multiple Email Notifications. These Notifications can be conditional, have different templates (body content) and recipient settings.

If your notifications are fine now, you do not have to do anything else with them.

Please let us know if something is not clear. We would be happy to help.
