Dual List Box Widget displaying on form in preview mode, not showing in edit mode

I added the Dual List Box Widget to my form and then removed it by hovering my cursor over the widget and selecting the remove button.

When I view my form in Build mode the Dual List Box no longer appears.  However, as soon as I preview the form two 'Dual List Boxes' appear at the end of the final page (see picture).  It doesn't matter if I remove the last page or add a new final page, the two boxes keep appearing on the very last page of the form.

Can someone help me remove this?

I have checked your JotForm in edit mode and notice that the 'Dual List Box' widgets are still existing on the form. 

I have now removed from the form and they are not displayed in the preview mode now. Please check and let us know if you need any further assistance. We will be happy to assist.
