What would you recommend for 3 fields side by side with an option to dynamically add another row?

What would you recommend for 3 fields side by side with an option to dynamically add another row? So I'm basically looking for:

First Name | Surname | Number 

with the option to add as many rows as needed. Can this be done with a widget or simply with logic?
